Advocating for access
The Assertive Outreach team is an essential element of Haven’s aged and disability services, linking people to our Community Connections and Housing Support for the Aged programs.
Six experienced and qualified staff work within the community and with various agencies to identify and support people whose needs are often overlooked by mainstream services. This includes those who are experiencing multiple and complex health problems and who are homeless, living in inappropriate or unsafe housing or at risk of becoming homeless.
Haven’s Assertive Outreach Service is an Aged Care and Home and Community Care (HACC) funded program, supported by both the state and federal governments

Our locations
The Assertive Outreach service operates across the Central Goldfields Shire, City of Greater Bendigo, Macedon Ranges Shire, Loddon Shire and Mount Alexander Shire. We also have a staff member in Robinvale who works with Mallee region communities.
Our programs
Haven’s two outreach programs are:
- Community Connections Program, and
- Housing Support for the Aged Program
Both programs focus on improving the health and wellbeing of clients to assist them to live independently in the community for as long as possible.
