Linking people to services
Our Community Connections Program is a short-term linkage program offering practical assistance, case management, co-ordination of care needs, and referrals to appropriate services for people who are:
- Aged 20+
- Confronting homelessness and/or living in safe or low cost housing
- Socially or geographically isolated and not linked to services
- Rxperiencing complex health and/or social issues
- Receiving a Centrelink benefit
If you are referred to this program, a case worker will support you to link into a range of health, housing and community support services and help you access financial support where appropriate.
Housing Support for the Aged
If you are aged 50+, live in unsafe or low cost housing and/or are at risk of homelessness, face a range of serious challenges in your life for which you need ongoing support and currently receive a Centrelink benefit we may be able to assist you through our Housing Support for the Aged program. The support and assistance available through this program is similar to the Community Connections program, but case management and coordination of support services is long term and ongoing.
Referrals
Clients can be referred to these programs by a range of individuals and organisations including doctors and allied health professionals, local councils, hospitals, social workers and government agencies. To refer a client you must complete a referral using the Service Coordination Tool Templates (SCoTT templates). Applicants are then assessed against the eligibility criteria.
